At the start of the movie, Landon and his friends challenge a student named Clay Gephardt Matt Lutz with an initiation task so he can join their popular friend group. They trick Clay into diving into a lake, but Clay is severely injured. Landon then jumps into the lake to rescue Clay. Landon gets poop in his pants at first, and Jamie tells him the reason she did not tell him is that she was moving on with her life and using the time she had left but then Landon happened and she fell in love with him.

Throughout the book, Landon emphasizes that he still waits for a miracle to happen. He mentioned that alot of times in the book and I like to think he really did get his miracle that's why forty years later he couldn't find a reason to remove his wedding ring because Jamie lived through.
His memory of her wouldn't be as painful as it would be for anyone who lost a love one if they were actually still together. The author mentioned that he didn't really want to end the book with Jamie suddenly survived the illness so he wanted to do it discreetly by writing "miracles can happen" because he didn't really want her to die.
So yup, I choose to think Jamie lived and grew old with Landon. This also made me remember the part where Jamie smiled at Landon when she first sighted him in the drama class and the part where Jamie asked him not to fall in love with her.
It's like everything has been already set to place and that they were only waiting for the right time for things to happen. Just like how Landon was very persistent of waiting for a miracle. If Landon never met Jamie he probably would have failed in School and never would have gotten a Job.
But after Jamie dies Landon has a good job and also got to marry a women he didn't get to know very well but he loved her from all his heart.

Jamie was dying, and the life she lacked all that she and everyone deserves. She wanted to marry for love. And all that came into her life in form of Landon. Landon was her miracle. She didn't live, she died as she was supposed to but before she did, she got whatever she wished for except a longer life.
Even her father comes close to Landon, who he considered a trouble in the beginning. Also, Landon get a chance to come close to his own dad, for whom he didn't have much of an opinion. At last Landon changed as a man. From a bully he turned into a caring person. Now, not much an happy ending since the leading female characters is dead it's too hard for the authors to kill someone directly , but there are reasons to settle with that ending in a non-unhappy manner.
I thought it was clear she died. His realization of the fact that he loved her was completely overwhelming to him and surprising. He had a moment of complete clarity; it was the understanding that he had a chance to have a relationship and love that many don't ever have.
Maybe I prefer to think his dedication is unending no matter what happens. He doesn't feel like he lost anything. He feels like he had a very fulfilling relationship.

Jamie reveals it to him that she has been suffering from Leukemia for two years and has now stopped responding to treatment. This, of course, leaves Jamie with two choices—he can either drift back to his old life or he can make the best of his limited time with Jamie.
She claims that before Landon came along, she had already accepted what God had in store for her. But for once, after falling for Landon, she was being forced to be mad at God. But even after questioning her own beliefs, Jamie manages to get a grip on them.
